Your full name so we can sign off communications properly.

Their last name so we can address them properly in any outreach.

Used to send the feature notification if you choose Option A or B below.

The official name of their company or practice.

CLSP-L14 — Approximately when did this client first come to you?

Month and year is fine. e.g. March 2023. This gives context for how long the relationship has been going.

CLSP-L15 — What service or solution did they initially come to you for?

Just the starting point — not everything you've done together. What was the first thing that brought them to you?

CLSP-L16 — What industry or niche is this client in?

e.g. HVAC contractor, restaurant owner, physical therapist, accountant. Be specific.

CLSP-L17 — On a scale of 1–10, how likely are they to enthusiastically participate in a recorded interview or on-camera feature?

1 = no way, 10 = they'd love it and do it tomorrow. This helps us recommend the right path for featuring them.

CLSP-L18 — Do you have permission to share their story publicly with their name attached?

If not, we can build the content with anonymized details. Just let us know where you stand.

CLSP-L19 — Do you have any existing photos, results data, screenshots, or documentation of their transformation? Where is it?

Google Drive link, phone photos, email threads with results, screenshots of wins — anything we can reference when building the content.

CLSP-L20 — Would you like to notify your client that you're featuring them?

Involving your client almost always produces better content AND strengthens your relationship with them. Choose the option that fits best.

Option A — Yes. Send them a warm email letting them know we want to shine a spotlight on them. We'll send them a short form to fill out and build the content ourselves. Low lift for them, strong result for you.

Option B — Yes, and go bigger. Do everything in Option A, plus invite them to a 30–60 minute recorded conversation or on-camera interview. This produces the richest possible content — YouTube videos, short clips, reels, a full case study page, and social proof their own audience will want to share.

Option C — No. Create the content internally using what I know, without notifying them. We'll keep details appropriately anonymous if needed.
